I've just requested a payoneer card from Elance using my paid freelance account. That card has quite good rates:

• Card activation (Outside U.S.) - $10
• Monthly account maintenance - $1 (2+ transactions per month)
• ATM Withdrawal (Outside U.S.) - $2.15

I don't quite sure if I'll be able to attach that card to another payoneer partners accounts, but it will be nice to use the card even with Elance only.

Elance also doesn't allow to attach non-elance card to a freelancer's account. You can only request a card with Elance logo and withdraw money to it.
